About the position
The position involves interpreting physician referrals and completing evaluations in various areas including musculoskeletal, neuromuscular, cardiopulmonary, and integumentary systems. The individual will establish a plan of care based on evaluation results and functional goals, monitor and evaluate treatment outcomes, and adjust treatment plans as necessary. Responsibilities also include planning for patient discharge, providing written home programs, and collaborating with stakeholders to ensure optimal patient care. Documentation of patient care, maintaining productivity standards, and adhering to billing policies are also key responsibilities. The role requires participation in performance improvement activities and professional growth through continuing education. Supervision of physical therapy assistants and students may also be part of the role.
Responsibilities
- Interprets physician referrals and completes evaluations in musculoskeletal, neuromuscular, cardiopulmonary, and integumentary areas.
- Establishes and implements individual treatment plans based on evaluations and functional goals.
- Monitors and evaluates treatment outcomes, adjusting plans as necessary.
- Plans and completes patient discharge, providing home programs and referrals.
- Communicates and collaborates with stakeholders for optimal patient care.
- Completes required documentation following department policy and accreditation standards.
- Maintains productivity standards and adheres to billing and documentation policies.
- Cleans up after patient treatment and keeps the area orderly.
- Participates in performance improvement activities.
- Pursues professional growth through continuing education and departmental in-services.
- May supervise physical therapy assistants and students.
Requirements
- Doctorate degree in Physical Therapy from an accredited school.
- Physical Therapy License in the State of Illinois or Wisconsin.
- Basic Life Support (BLS) certification for Healthcare Providers.
- No prior work experience required.
- Intermediate computer skills.
- Excellent communication, interpersonal, and organization skills.
- Demonstrated organization, decision-making, and problem-solving skills.
- Knowledge of age-specific care requirements.
