About the position
The Social Work Care Manager at AdventHealth plays a crucial role in addressing the complex psychosocial needs of patients. This position involves assisting patients with eligibility determination for social programs, coordinating discharge planning, and providing crisis intervention. The role is integral to ensuring that patients receive the necessary support and resources during their care journey, particularly for high-risk populations.
Responsibilities
- Intervene with patients who have complex psychosocial needs.
- Assist with eligibility determination for social programs and funding sources.
- Provide crisis intervention to patients and families with psychosocial needs.
- Coordinate and facilitate the development of a discharge plan of care for high-risk patient populations.
- Receive referrals for individuals from at-risk populations from interdisciplinary team members.
- Conduct psychosocial assessments and reporting interventions in cases of abuse and neglect.
- Provide consult services for patients lacking decision-making capacity and coordinate with Care Management leadership.
- Offer consult services for foster care and adoptions.
Requirements
- Masters in Social Work (MSW)
- Minimum three (3) years experience in hospital/medical social work.
Nice-to-haves
- Care Management discharge planning experience
- Knowledge of state and federal guidelines pertinent to care management
- Licensed Clinical Social Worker (LCSW)
- ACM/CCM certification
