About the position

The Pediatric Physical Therapy Assistant at Circle of Care plays a crucial role in delivering compassionate, patient-centered healthcare to children from birth to age 20. This position involves assisting in the implementation of rehabilitative care plans under the supervision of a qualified physical therapist in a home health setting, ensuring that patients receive tailored support to enhance their physical abilities and overall well-being.

Responsibilities

  • Create and update individualized patient care plans to address physical therapy needs, in alignment with the direction of the supervising PT.
  • Evaluate patients by accurate diagnosis and prognosis, and treat patients to alleviate pain, improve function, and enhance their ability to perform at maximum potential, in alignment with the supervising PT.
  • Create and maintain patient records and clinical notes in a timely manner.
  • Attend and participate in case conferences with local teams monthly, and actively participate in staff development opportunities.
  • Communicate between supervising physical therapist, agency, clients, and your Area Support Manager to ensure a well-coordinated approach to patient care.

Requirements

  • Associate's degree from an accredited Physical Therapist Assistant Program.
  • Current state licensure or certification as a Physical Therapist Assistant.
