About the position

The Peer Counselor position at Concern Housing involves providing direct support and services to clients from a peer perspective. The role emphasizes person-centered care and aims to assist clients in their recovery journey by offering various services, including counseling, skill teaching, and support group facilitation. The Peer Counselor will work within a Mobile Residential Team, contributing to the overall well-being and empowerment of participants.

Responsibilities

• Provides direct care services to clients.
• Responsible for documentation of clients assigned to as well as documentation relating to other clients' progress.
• Facilitates support and educational groups for clients.
• Educates staff about peer perspective, person-centered language, rights and protections, and alternatives.
• Assures that consumers know their rights and responsibilities and assists them in identifying options and participating in decisions related to their goals.
• Provides rehabilitative support services in daily living skills, health services, and others as assigned.
• Provides Wellness Self-Management training and assists participants in developing skills and strategies that promote recovery.
• Participates in program meetings and attends supervision regularly.
• Accompanies consumers to appointments if needed to provide additional support and understanding.
• Remains current with all Agency required compliance documents and trainings.

Requirements

• Must have a High School Diploma and peer support credentials.
• Valid driver's license is a must.
• Must have familiarity with benefits and entitlements.
• Ability to communicate effectively and guide others through the self-help process.
• Experience with travel training, facilitating peer support & educational groups, and/or assisting others with ADL skills required.
