About the position

The Bridge is an innovative and well-respected mental health agency serving adults with serious behavioral health diagnoses, substance use issues, and co-occurring medical conditions. The Bridge envisions a world where adults with mental illness can thrive in the Community. Our mission is to change lives by offering help, hope, and opportunity to vulnerable adults in New York City. We work from a Harm Reduction, Trauma Informed perspective and acknowledge the impact that systemic racism has had on the communities we serve. The CTC program provides comprehensive, coordinated, and evidenced-based services for individuals, youth, and families with a serious mental illness (SMI), serious emotional disturbance (SED) or co-occurring disorder (COD) who are experiencing homelessness or at imminent risk of homelessness. This is a community-based program. Services take place where individuals experiencing homelessness spend time, on the street, in the subway system, in a drop-in center, or shelter. This is a community-based program where 80% of the work is in the communities where clients live and spend time.

Responsibilities

  • Collaborate with a team to provide individuals with person-centered peer support services related to serious mental illness (SMI), serious emotional disturbance (SED) or co-occurring disorder (COD).
  • Complete required documentation in a timely and high-quality manner.
  • Work collaboratively with clients, their families, and community partners to assist clients in achieving their treatment and recovery goals.
  • Coordinate client care through close involvement with hospitals, substance use treatment facilities, probation and parole, and other community partners.
  • Complete psychosocial, substance abuse, and other treatment-related assessments.
  • Collaborate with the client in the creation of individualized service plans, and crisis/relapse prevention plans.
  • Use evidence-based practices (EBPs) in the areas of substance use and mental health to address client needs (WRAP, harm reduction, etc.).
  • Provide services to assist clients with needs such as family support, housing, entitlements, employment, education, vocational interests, community integration, and connection to physical health care.
  • Provide services to assist clients with needs such as legal advocacy, and navigating the criminal justice system in coordination with legal agencies.

Requirements

  • Certified Peer Specialist or Certified Recovery Peer Advocate.
  • Excellent verbal and written language skills.
  • Bilingual Spanish speaking a plus.
  • Computer skills.
  • Must have lived experience in substance use and/or behavioral health.
