About the position

The ACT Peer Support Specialist plays a crucial role in a community-based mental health team, providing support to individuals experiencing serious mental illness and substance use disorders. This position focuses on mentoring and assisting clients in achieving community inclusion, independence, and recovery. The specialist will deliver at least 80% of services in the beneficiaries' homes or other community settings, facilitating wellness groups and connecting clients to necessary resources.

Responsibilities

  • Support individuals in acquiring and practicing daily life activities.
  • Connect individuals to necessary services including DHS, Social Security, and community resources.
  • Facilitate wellness groups such as WRAP and WHAM, and assist individuals in accessing community programs.
  • Promote client decision-making and participation in the person-centered planning process.
  • Assist individuals with implementing their goals identified in the PCP.
  • Attend daily ACT team meetings to review beneficiary status and care management needs.
  • Perform a minimum of 80% of ACT contacts in the beneficiary's home or community location.
  • Attend outings to enhance socialization and provide instructional support to clients.
  • Utilize a Trauma-Informed approach to deliver services.
  • Complete required documentation accurately and in accordance with CNS policy.
  • Fulfill mandatory education requirements and maintain compliance with quality care initiatives.
  • Provide transportation assistance to individuals served as needed.
  • Participate in on-call rotation as assigned.
  • Ensure compliance with established policies and procedures.

Requirements

  • High school diploma or equivalent.
  • State of Michigan certification as a Certified Peer Support Specialist (CPSS) or Certified Peer Recovery Coach (CPRC).
  • Valid and unrestricted Driver's License.
  • Transportation or automobile accessibility with current insurance meeting agency policy requirements.
  • Participation in MDHHS-approved ACT-specific training within 6 months of hire and annually thereafter.
  • Proficient with computers and Microsoft Office products.
  • Ability to work as part of a multidisciplinary team and interact professionally with clients.

Nice-to-haves

  • 1 year paid or volunteer experience working with adults with severe and persistent mental illnesses.
  • Prior experience as a peer support specialist.
  • Experience with electronic health record (EHR) systems.
