About the position

The Peer Support Specialist - Integrated Health Home at UnityPoint Health is responsible for providing support services to adults with Serious Mental Illness (SMI) through a patient-centered approach. This role emphasizes the importance of integrated care, helping members in their recovery process while serving as an advocate. The specialist will utilize problem-solving techniques, teach coping skills, and facilitate recovery dialogues to empower members in achieving their personal health goals.

Responsibilities

  • Utilizes and teaches problem-solving techniques with individuals and groups.
  • Teaches consumers how to identify and combat negative self-talk and overcome fears.
  • Teaches and role models the value of every individual's recovery experience.
  • Leads and teaches consumers how to facilitate recovery dialogues and/or self-management plans.
  • Documents services using the Center's Electronic Health Record and other Provider Portals as required.
  • Completes health assessments and other requirements outlined by funders.
  • Maintains patient confidence and protects operations by keeping information confidential.
  • Participates in meeting performance measures and continuous quality improvement activities, including data collection, tracking, and analysis.
  • Adheres to all policies and procedures set forth by UnityPoint Health and other funding sources.
  • Accomplishes all tasks as appropriately assigned or requested.
  • Develops and assists with monitoring care coordination plans.
  • Promotes patient's health and ensures that all personal health goals are included in person-centered care management plans.
  • Conducts home visits, contacts members by phone, and attends meetings related to enrolled members.

Requirements

  • High School Diploma or Equivalent
  • Lived experience with a serious mental illness with demonstrated stability in recovery.
  • Valid driver's license
  • Complete Peer Support Training
  • Write, read, comprehend, and speak fluent English.

Nice-to-haves

  • Experience in working with adults with mental illness.
  • Certified as a Peer Support Specialist or Peer Recovery Specialist through the Iowa Board of Certification.
  • Knowledge of community resources.
