About the position

The Transition Coordinator for Indiana's Department of Correction: Division of Youth Services is responsible for overseeing transition-related practices in juvenile correctional facilities. This role involves implementing life skills curricula, evaluating transition consultations, participating in administrative release committees, and assisting youth in their reentry into the community. The position requires collaboration with various agencies and organizations to support youth in obtaining necessary documentation and transitioning back to public schools or employment opportunities.

Responsibilities

  • Facilitate the implementation of a life skills curriculum, titled Next Steps.
  • Utilize the Transition Coordinator Handbook to standardize transition practices.
  • Evaluate the five 'Transition Consultations' for every student under the guidance of an assigned teacher.
  • Participate weekly in Administrative Release Committees (ARC) to confirm and reinforce student reentry plans.
  • Conduct follow-up phone calls to released youth at 1, 3, 6, 9, and 12 months post-release.
  • Represent the Indiana Department of Correction- Division of Youth Services at conferences and meetings.
  • Assist in developing partnerships with Indiana's Department of Workforce Development, HIRE, and Indiana University's HOPE Mentoring.
  • Help develop individualized Education Learning Plans for each youth upon entry at the facility.
  • Work with state, county, and private agencies to provide re-entry services for youth transitioning back into the community.
  • Assist students in obtaining social security cards, state identification, driver's licenses, birth certificates, and additional documentation.
  • Support students in transitioning back to public schools and provide information on employment, apprenticeship programs, and military service.

Requirements

  • Valid Driver's License.
  • Experience in supervising youth.
  • Master's degree.
