About the position

The Transition Coordinator for Indiana's Department of Correction: Division of Youth Services is responsible for overseeing transition-related practices in juvenile correctional facilities. This role involves implementing life skills curricula, standardizing transition practices, and supporting youth in their reentry into the community.

Responsibilities

• Facilitate the implementation of a life skills curriculum, titled Next Steps.
• Utilize the Transition Coordinator Handbook to standardize transition practices.
• Evaluate the five 'Transition Consultations' for every student under the guidance of an assigned teacher.
• Participate weekly in Administrative Release Committees (ARC) to confirm and reinforce students' reentry plans.
• Conduct follow-up phone calls to released youth at 1, 3, 6, 9, and 12 months post-release.
• Represent the Indiana Department of Correction- Division of Youth Services at conferences and meetings.
• Assist in developing partnerships with Indiana's Department of Workforce Development, HIRE, and Indiana University's HOPE Mentoring.
• Help develop individualized Education Learning Plans for each youth upon entry at the facility.
• Work with state, county, and private agencies to provide re-entry services for youth transitioning back into the community.
• Assist students in obtaining necessary documentation such as social security cards, state identification, and birth certificates.
• Support students in transitioning back to public schools and provide information on employment, apprenticeship programs, and military service.

Requirements

• 1 year of supervising experience
• 1 year of social work experience
• 1 year of management experience

Benefits

• Contract position with hourly pay of $20.93 - $21.00
• 8 hour shift from Monday to Friday
