About the position

The Nurse Practitioner (NP) position at Optum Home & Community Care focuses on providing care to patients in various settings, including nursing homes and assisted living facilities. The role involves conducting comprehensive assessments, ensuring smooth transitions from rehabilitation to home, and preventing readmissions. NPs will work independently, communicating findings to attending staff without the burden of managing care directly. Responsibilities

• Perform comprehensive assessments and document findings in compliance with documentation requirements and CMS regulations.
• Ensure all diagnoses are accurately coded using ICD10 and documented appropriately.
• Address and document all quality elements during patient visits.
• Conduct initial medication reviews.
• Travel between care sites as required.
• Communicate with patients and team members regarding medical concerns during visits.
• Participate in program enhancement initiatives.

Requirements

• Certified Nurse Practitioner through a national board.
• Graduate of an accredited master's degree in nursing (MSN) program.
• Board certified through AANP or ANCC, with preferred certification as ANP, FNP, or GNP.
• Active and unrestricted license in the state of residence.
• Ability to lift a 30-pound bag and navigate various dwelling conditions.
• Availability to work 10 hours per week.
• Access to reliable transportation for travel to client sites.

Nice-to-haves

• 1+ years of hands-on post-graduate experience within Long Term Care.
• Understanding of Geriatrics and Chronic Illness.
• Understanding of Advanced Illness and end-of-life discussions.
• Proficient computer skills for documenting medical information.
• Proven ability to develop and maintain positive customer relationships.
• Proven adaptability to change.
