About the position

Providence ElderPlace Oregon is seeking a Geriatric Physician for an On-Call/Per Diem position. This role involves joining an established interdisciplinary team dedicated to providing comprehensive care and social services to older adults. The physician will collaborate with various healthcare professionals to ensure coordinated care for seniors, emphasizing a commitment to diversity, equity, and inclusion.

Responsibilities

• Provide comprehensive medical care to older adults as part of an interdisciplinary team.
• Collaborate with nurse care managers, social workers, pharmacists, and mental health case managers.
• Participate in the coordination of care services for seniors within the PACE model.
• Engage in ongoing education and training, including obtaining MAT Waiver for prescribing Suboxone.

Requirements

• Board-certified or board-eligible in Family Medicine or Internal Medicine.
• Completion of a Geriatric Medicine fellowship is preferred.
• Interest in working with older adults and providing coordinated care.

Nice-to-haves

• Experience in geriatric medicine or related fields.
• Familiarity with the PACE model of care.
