About the position

The LVN Triage Scheduler plays a crucial role in supporting the Triage Staff by performing a variety of clerical and clinical duties. This position involves monitoring staff schedules, managing assignments, and providing phone support, all while applying expert clinical judgment to ensure high-quality patient care. The role is essential for coordinating resources effectively and minimizing overtime for staff, contributing to the overall mission of The Elizabeth Hospice to enhance the quality of life for patients and their families.

Responsibilities

  • Coordinate, prioritize, and communicate daily LVN assignments in collaboration with RN Case Managers and Supervisors.
  • Assist with daily schedule assignment and reassignment while minimizing staff overtime.
  • Daily coordination of Crisis Care scheduling and communication in collaboration with Triage RNs and Supervisors.
  • Submit visit requests for follow-up on Quality Measures from Thursday/Friday admissions for Saturday/Sunday.
  • Update appropriate boards including LVN Assignment Board, Crisis Care Assignment Board, Monthly Schedule, and Emergency On Call List.
  • Coordinate with Admissions/Triage staff for scheduling of Crisis Care/Tuck In's throughout the workday.
  • Assist in preparation of Monthly Clinical Staff Calendar and communicate coverage of open shifts.
  • Assist Triage PCC in creating schedule templates with Supervisor approval.
  • Provide assistance with overflow phone calls from Clinical Support and Triage RNs.
  • Offer computer support and training to staff as needed.
  • Support oversight of staff needing access to Supply Room when DME clerk is unavailable.
  • Perform special projects or other assignments as requested by Manager or Triage Supervisors.

Requirements

  • Licensed Vocational Nurse (LVN) with applicable licensure and experience.
  • Strong clinical judgment and knowledge to coordinate patient care effectively.
  • Excellent communication and organizational skills.

Nice-to-haves

  • Experience in hospice or palliative care settings.
  • Familiarity with scheduling software and tools.
