About the position
The Per Diem LVN position at Calstro Hospice involves providing care for terminally ill patients in their homes under the supervision of a Registered Nurse (RN). The role requires adherence to individualized care plans, including medication administration, patient and family education on end-of-life processes, and ensuring safety in the home environment. The LVN must be organized, possess strong communication skills, and document care accurately while following company policies.
Responsibilities
- Care for terminally ill patients in their homes under RN supervision.
- Implement and educate patients and families on end-of-life processes.
- Administer medications and treatments as per MD orders.
- Ensure safety in the home environment.
- Communicate effectively with administrative and field staff.
- Document care appropriately and follow company policies and procedures.
Requirements
- Currently licensed as an LVN in the State of California.
- Three years of nursing experience.
- One year of prior experience as a hospice LVN.
- Current BLS provider card.
- Great verbal and written communication skills.
- Strong organizational skills.
- Must be a licensed driver with an insured automobile.
- Basic computer competence.
