About the position

Advocate Aurora Health is seeking a per diem / prn Float Physical Therapist in Hartford, Wisconsin. This role involves providing physical therapy services, interpreting physician referrals, and developing individualized treatment plans based on patient evaluations. The therapist will monitor patient progress, adjust treatment plans as necessary, and ensure proper documentation of care. The position requires a commitment to patient comfort and effective communication with both patients and healthcare teams.

Responsibilities

  • Interprets physician referrals and completes evaluations in various disciplines.
  • Establishes and implements individual treatment plans based on evaluations and functional goals.
  • Monitors and evaluates treatment outcomes, adjusting plans as necessary.
  • Identifies and addresses factors affecting patient comfort.
  • Plans and completes patient discharge, including home programs and referrals.
  • Completes required documentation following department policy.
  • Applies insurance guidelines regarding authorization and visits.
  • Maintains cleanliness and orderliness of treatment areas.
  • Performs manual techniques and patient positioning duties, utilizing proper techniques.
  • Demonstrates skill in the use of modalities and exercise equipment.
  • Participates in performance improvement activities to enhance patient care outcomes.
  • Manages assigned caseload and informs leadership of coverage needs.
  • Pursues professional growth through continuing education and literature.
  • Orients new staff to clinical and department policies.
  • Participates in the evaluation process of co-workers.

Requirements

  • Basic Life Support (BLS) certification within 6 months of hire.
  • Physical Therapist license issued by the state of Wisconsin.
  • Bachelor's Degree in Physical Therapy.
  • No prior experience required.
  • Intermediate computer skills.
  • Excellent communication and interpersonal skills.
  • Demonstrated organization, decision-making, and problem-solving skills.
