About the position

The Quality Management Specialist is responsible for leading performance improvement initiatives within Mount Carmel Home Care. This role involves collaborating with local agencies and leadership to develop, implement, and evaluate quality improvement programs, ensuring compliance with agency licensure, accreditation, and regulatory requirements. The specialist focuses on enhancing clinical outcomes, operational efficiency, and electronic medical record (EMR) documentation, while providing guidance to clinical staff and agency leadership to support patient-centered care and fiscal goals.

Responsibilities

  • Lead agency-wide QA/QI and process improvement initiatives aimed at achieving top-tier patient outcomes, satisfaction, and compliance with strategic goals.
  • Ensure adherence to federal, state, and accreditation standards through record reviews, case conferences, and survey preparedness.
  • Oversee QAPI (Quality Assurance/Performance Improvement) reporting.
  • Facilitate in-service training on quality improvement and regulatory compliance.
  • Use EMR and publicly reported data to track quality performance.
  • Review clinical documentation for accuracy and compliance with regulations, recommending improvements and ensuring proper documentation during audits and payer relations.
  • Lead infection control, safety, and patient satisfaction initiatives, ensuring adherence to protocols.
  • Design and implement QAPI programs and action plans to meet strategic goals and ensure regulatory compliance.
  • Maintain survey readiness and lead accreditation efforts, ensuring successful survey outcomes.
  • Demonstrate independent judgment in quality initiatives and ensure efficient, effective operations.
  • Mentor clinical staff to improve performance and adherence to best practices, including documentation accuracy.
  • Work closely with leadership and cross-functional teams to achieve quality goals and enhance patient outcomes.

Requirements

  • Valid RN Bachelor's degree required.
  • Minimum of four years' experience in home care, with at least two years in a supervisory role preferred.
  • Strong communication and interpersonal skills.
  • Knowledge of quality improvement principles and data analysis.
  • Proficiency in computer programs and videoconferencing software.
  • OASIS certification (COS-C or HCS-O) within six months of hire, with ongoing certification maintenance.
  • Valid Driver's License and reliable transportation.
