About the position

The Perinatal Case Manager will work in Baton Rouge and Lafayette to connect pregnant women diagnosed with syphilis, HIV, and other communicable diseases to medical care and community-based supportive services. This role involves ensuring treatment access and adherence, as well as educating healthcare providers on screening guidelines and treatment protocols. The position requires strong community engagement and program implementation skills, along with a solid understanding of maternal and child health.

Responsibilities

  • Connect pregnant women diagnosed with syphilis/HIV to medical care and community-based supportive services.
  • Ensure treatment access and adherence for patients.
  • Educate healthcare providers on STI/HIV screening guidelines and treatment protocols.
  • Facilitate community engagement and program implementation.
  • Conduct quality assurance activities related to case management.

Requirements

  • Licensed Practical Nurse (LPN), Registered Nurse (RN), or Licensed Clinical Social Worker (LCSW).
  • At least 2 years of experience in in-home health and/or social services delivery, medical case management, or treatment adherence programs.
  • General knowledge of communicable disease transmission, maternal and child health, and health education.

Nice-to-haves

  • At least one year of professional experience in maternal/child health.
  • Experience working with communicable diseases.
