About the position

The Perinatal Case Manager will work in Monroe, LA, under Acadiana CARES in collaboration with the Louisiana Office of Public Health. The primary role is to connect pregnant women diagnosed with syphilis, HIV, or other STIs to necessary medical care and community support services. This position also involves educating healthcare providers on screening guidelines and treatment protocols, ensuring that patients have access to treatment and adhere to their care plans. The role requires strong community engagement and program implementation skills, along with a solid understanding of communicable diseases and maternal and child health.

Responsibilities

  • Connect pregnant women diagnosed with syphilis/HIV to medical care and community-based supportive services.
  • Ensure treatment access and adherence for patients.
  • Educate healthcare providers on STI/HIV screening guidelines and treatment protocols.
  • Implement programs and facilitate community engagement activities.
  • Conduct quality assurance to ensure effective service delivery.

Requirements

  • Licensed Practical Nurse (LPN), Registered Nurse (RN), or Licensed Clinical Social Worker (LCSW) certification.
  • At least 2 years of experience in in-home health and/or social services delivery, medical case management, or treatment adherence programs.
  • General knowledge of communicable disease transmission, maternal and child health, and health education.

Nice-to-haves

  • At least 1 year of professional experience in maternal/child health and working with communicable diseases.
