Our mission is to empower clients to retain their independence by providing essential personal care services in the comfort of their own homes. We are currently seeking a compassionate and reliable Personal Care Assistant to join our growing team. In this role, you will collaborate closely with the case manager to follow each client's service plan, ensuring that their care goals are met effectively. This includes providing personal care services, assistance, support, and supervision as needed. Responsibilities

• Work collaboratively with the case manager to follow the client's service plan.
• Provide personal care services to clients in their homes.
• Assist clients with daily living activities and personal hygiene.
• Support and supervise clients to ensure their safety and well-being.
• Maintain client confidentiality and adhere to HIPAA regulations.
• Document care provided and report any changes in client condition to the case manager.

Requirements

• High school diploma or GED required.
• Minimum one (1) year of documented caregiver or home care assistance experience is required.
• Certified CPR and negative TB skin test or chest x-ray required.
• Licensed driver with an insured and reliable automobile and proof of insurance.
• Ability to read, write, speak, and understand English and communicate effectively.
• Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull as needed for client care.
• Must be able to lift up to 35 pounds.
