About the position

The Personal Care Worker is responsible for providing essential care services to support the personal needs and health status of participants, primarily in a facility setting with occasional home care assignments. This role involves working as part of an interdisciplinary team to assess and monitor participants, ensuring their independence in daily activities and maintaining their overall well-being.

Responsibilities

  • Participate as a member of the interdisciplinary team in ongoing participant assessment and monitoring.
  • Report on personal care services provided and the self-care abilities of participants, including changes noted in their condition.
  • Provide personal care assistance to maintain the independence of participants in daily living activities such as toileting, showers, eating, and grooming.
  • Assist in maintaining physical activity, exercise, and walking for participants.
  • Provide nursing care as assigned in a safe manner, consistent with the policies and procedures of the PACE Organization.
  • Identify safety hazards and emergency situations, and initiate corrective action as able.
  • Provide behavioral supervision or support for participants, including reality orientation and emotional reassurance.
  • Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.
  • Identify signs and symptoms indicating physical, emotional, or behavioral changes in participants and report these to the center staff and/or supervisor.
  • Ensure compliance with infection control, personal protection equipment, and safety standards.
  • Escort participants to medical, dental, optometry, rehabilitation, and other appointments as necessary.
  • Attend staff meetings and participant care planning regularly.
  • Assist transportation staff with loading and unloading participants from vehicles.
  • Maintain current written records of services provided and participant's expressed wishes.

Requirements

  • High school diploma or equivalent required.
  • Certified nurse assistant certification in the state preferred.
  • Minimum of one (1) year of documented experience working with a frail or elderly population, or appropriate training from the PACE organization upon hiring.
  • Experience in a long-term care facility or community-based geriatric program preferred.
  • Action-oriented with business acumen and conflict management skills.
  • Customer-focused with high decision quality and flexibility to adapt to ongoing change.
  • Ability to work with minimal supervision and exercise independent judgment.
  • Demonstrates superior written, verbal, and/or American Sign Language (ASL) communication skills.
