About the position

The Personal Care Assistant at LIFE Geisinger provides essential personal care and support to participants in both the Adult Day Health Center and their homes. The role focuses on maintaining the independence and health of older adults through direct care, homemaking services, and restorative care, all while adhering to care plans developed by the home care coordinator. This position requires effective communication, attention to safety, and collaboration with an interdisciplinary team to ensure quality care.

Responsibilities

  • Provide personal care assistance to participants at the Adult Day Health Center and in their homes.
  • Assist with personal care, nutrition, hydration, and obtain vital signs.
  • Help with ambulation and transfers of participants, including active and passive range of motion exercises.
  • Prompt participants for medication compliance and manage incontinence care.
  • Perform light housekeeping and assist with laundry as needed.
  • Deliver medication and supplies to participants' homes.
  • Maintain safety by using proper body mechanics and equipment like gait belts and back braces during transfers.
  • Collect data for quality improvement projects and participate in action plans to enhance care quality.
  • Document services provided accurately according to program policy.
  • Communicate effectively with participants and co-workers, explaining actions before starting tasks.
  • Promote independence in participants by offering choices and fostering self-help skills.
  • Attend interdisciplinary team meetings and complete required educational training.

Requirements

  • High School Diploma or Equivalent (GED) required.
  • Minimum of 1 year related work experience preferred.
  • Cardiopulmonary Resuscitation Certification from the American Heart Association (AHA) required.
  • Basic Life Support Certification required.
  • Valid Driver's License required.

Nice-to-haves

  • Experience working with older adults or in a healthcare setting.
  • Knowledge of home care practices and safety protocols.
