About the position

The Personal Care Assistant at LIFE Geisinger provides essential personal care and support to participants in both the Adult Day Health Center and their homes. This role focuses on maintaining the independence and well-being of older adults by delivering personalized care, homemaker services, and restorative care, all while adhering to care plans developed in collaboration with home care coordinators. The position emphasizes safety, effective communication, and the promotion of self-help skills among participants.

Responsibilities

  • Provide personal care assistance to participants at the Adult Day Health Center and in their homes.
  • Assist with personal care, nutrition, hydration, and obtain vital signs.
  • Support participants with ambulation, transfers, and range of motion exercises.
  • Prompt medication compliance and manage participant incontinence care.
  • Perform light housekeeping and assist with laundry as needed.
  • Deliver medication and supplies to participants' homes.
  • Maintain safety by using proper body mechanics and equipment during transfers.
  • Collect data for quality improvement projects and participate in action plans.
  • Document services provided accurately according to program policy.
  • Communicate effectively with participants and co-workers, explaining actions before tasks.
  • Promote independence by offering choices and fostering self-help skills.
  • Attend interdisciplinary team meetings and complete required training.

Requirements

  • High School Diploma or Equivalent (GED) required.
  • Minimum of 1 year of related work experience preferred.
  • Valid Driver's License required.
  • Cardiopulmonary Resuscitation Certification from the American Heart Association required.
  • Basic Life Support Certification required.
