About the position

The Personal Care Assistant at LIFE Geisinger provides essential personal care and support to participants, primarily seniors, in both a clinical environment and their homes. The role focuses on promoting independence and maintaining a healthy lifestyle for participants through direct care, homemaking services, and restorative care, all while adhering to care plans developed by home care coordinators. The position requires effective communication, attention to safety, and participation in quality improvement initiatives.

Responsibilities

  • Provide personal care assistance to participants at the Adult Day Health Center and in their homes.
  • Assist with personal care, nutrition, hydration, and obtain vital signs.
  • Help with ambulation and transfers of participants, including active and passive range of motion exercises.
  • Prompt medication compliance and manage participant incontinence care.
  • Perform light housekeeping and assist with laundry as needed.
  • Deliver medication and supplies to participants' homes.
  • Maintain safety by using proper body mechanics and equipment like gait belts and back braces during transfers.
  • Collect data for quality improvement projects and participate in action plans.
  • Complete accurate documentation of services provided according to program policy.
  • Communicate effectively with participants and co-workers, explaining actions before starting tasks.
  • Promote independence by offering choices and fostering self-help skills.
  • Attend interdisciplinary team meetings and complete required educational training.

Requirements

  • High School Diploma or Equivalent (GED) is required.
  • Minimum of 1 year of related work experience is preferred.
  • Valid Driver's License is required.
  • Cardiopulmonary Resuscitation (CPR) Certification from the American Heart Association is required.
  • Basic Life Support Certification is required.

Nice-to-haves

  • Experience working with older adults is preferred.
