About the position

The Personal Care Assistant at LIFE Geisinger provides essential personal care and support to participants in both the Adult Day Health Center and their homes. This role focuses on maintaining the independence and well-being of older adults by delivering personalized care, homemaker services, and restorative care, all while adhering to care plans developed in collaboration with home care coordinators. The position emphasizes safety, effective communication, and the promotion of self-help skills among participants.

Responsibilities

• Provide personal care assistance to participants at the Adult Day Health Center and in their homes.
• Assist with personal care, nutrition, hydration, and obtain vital signs.
• Help with ambulation and transfers of participants, including active and passive range of motion exercises.
• Prompt participants for medication compliance and manage incontinence care.
• Perform light housekeeping and assist with laundry as part of indirect participant care responsibilities.
• Utilize supplies accurately and economically, and deliver medication and supplies as needed.
• Maintain participant and personal care assistant safety by using proper body mechanics and equipment like gait belts and back braces.
• Collect data for mandatory requirements and quality improvement projects, participating in action plans to enhance program quality.
• Complete accurate documentation for services provided according to program policy.
• Communicate effectively with older adults and co-workers, explaining actions before starting tasks.
• Promote independence in participants by offering choices and fostering self-help skills.
• Attend interdisciplinary team meetings and complete required educational trainings.

Requirements

• High School Diploma or Equivalent (GED) is required.
• Minimum of 1 year of related work experience is preferred.
• Cardiopulmonary Resuscitation Certification from the American Heart Association is required.
• Basic Life Support Certification is required.
• Valid Driver's License is required.

• Experience working with older adults or in a healthcare setting is preferred.
