About the position

The Personal Care Assistant at LIFE Geisinger provides essential personal care and support to participants in both the Adult Day Health Center and their homes. The role focuses on maintaining the independence and health of older adults through direct care, homemaking services, and restorative care, all while adhering to care plans developed by home care coordinators. This position requires effective communication, attention to safety, and collaboration with an interdisciplinary team to ensure quality care.

Responsibilities

• Provide personal care assistance to participants at the Adult Day Health Center and in their homes.
• Assist with personal care, nutrition, hydration, and obtain vital signs.
• Support participants with ambulation, transfers, and range of motion exercises.
• Prompt medication compliance and manage participant incontinence care.
• Perform light housekeeping and assist with laundry as needed.
• Deliver medications and supplies to participants' homes.
• Maintain safety by using proper body mechanics and equipment during transfers.
• Collect data for quality improvement projects and participate in action plans.
• Document services provided accurately according to program policy.
• Communicate effectively with participants and co-workers, explaining actions before tasks.
• Promote independence by offering choices and fostering self-help skills.
• Attend interdisciplinary team meetings and complete required training.

Requirements

• High School Diploma or Equivalent (GED) is required.
• Minimum of 1 year of related work experience is preferred.
• Valid Driver's License is required.
• Cardiopulmonary Resuscitation Certification from the American Heart Association is required.
• Basic Life Support Certification is required.

Nice-to-haves

• Experience working with older adults is a plus.
• Knowledge of home care services and patient support.
