About the position

The Personal Care Assistant at LIFE Geisinger provides essential personal care and support to participants in both the Adult Day Health Center and their homes. This role focuses on maintaining the independence and well-being of older adults by delivering personalized care, assisting with daily activities, and ensuring a safe and healthy living environment. The position requires effective communication and collaboration with an interdisciplinary team to enhance the quality of care for participants.

Responsibilities

• Provide personal care assistance to participants at the Adult Day Health Center and in their homes.
• Assist with personal care, nutrition, hydration, and obtain vital signs.
• Help with ambulation and transfers of participants, including active and passive range of motion exercises.
• Prompt participants for medication compliance and assist with incontinence care.
• Perform light housekeeping and assist with laundry as needed.
• Deliver medication and supplies to participants' homes.
• Maintain safety by using proper body mechanics and equipment like gait belts and back braces during transfers.
• Collect data for quality improvement projects and participate in action plans to enhance care quality.
• Complete accurate documentation of services provided according to program policy.
• Communicate effectively with participants and co-workers, explaining actions before starting tasks.
• Promote independence in participants by offering choices and fostering self-help skills.
• Attend interdisciplinary team meetings and complete required educational training.

Requirements

• High School Diploma or Equivalent (GED) required.
• Minimum of 1 year of related work experience preferred.
• Valid Driver's License required.
• Basic Life Support Certification required.
• Cardiopulmonary Resuscitation Certification from the American Heart Association required.
