About the position

Liberty Healthcare and Rehabilitation Services is dedicated to providing compassionate care to our residents. We are currently seeking a Personal Care Assistant (PCA) to join our team on a per diem basis. The PCA will play a vital role in assisting residents with their daily living activities, ensuring their comfort and dignity at all times. This position requires a commitment to providing high-quality care and support to our geriatric patients, who rely on our staff for assistance with personal hygiene, mobility, and other essential tasks. As a PCA, you will be responsible for a variety of tasks that include assisting residents with bathing, dressing, and personal hygiene. You will also be required to change linens, take and record vital signs, and assist Certified Nursing Assistants (CNAs) with their duties. It is essential to follow facility policies regarding patient care, including turning and repositioning residents, maintaining cleanliness in patient units, and ensuring that all nursing equipment is properly cared for. You will also be responsible for completing patient care records accurately at the end of each shift, including inputting information into Patient Observation Records and Activity Attendance Records. In addition to these responsibilities, the PCA will assist with rehabilitation programs, provide post-mortem care when necessary, and ensure that residents have access to fresh water and clean linens. The role requires a compassionate approach, as you will be interacting with residents, their families, and other staff members. It is crucial to maintain a respectful and kind demeanor at all times, as well as to adhere to all safety protocols and facility policies. Responsibilities

  • Assists residents with bathing, dressing, personal hygiene, and activities of daily living (ADLs) as needed during shift.
  • Changes linen on beds according to bath schedule and/or as needed.
  • Takes and records vital signs per policy and/or as directed.
  • Assists CNAs with needed tasks.
  • Turns residents while in bed per facility policy.
  • Repositions residents while out of bed per facility policy.
  • Assists with oral hygiene including denture care when getting up in the morning, after meals, at bedtime, and/or when needed.
  • Answers call bells promptly following facility policy.
  • Completes patient care records at end of shift, including accurate intake and output (I and O).
  • Fills in Patient Observation Records and Activity Attendance Records for the next month at the end of the month.
  • Collects urine or stool specimens as directed.
  • Gives enemas as directed.
  • Keeps patient unit/halls clean and neat.
  • Cleans bedside tables, chest of drawers, and closets, including clothing.
  • Cares for and cleans nursing equipment such as BP equipment and thermometers.
  • Follows restraint policy, including release.
  • Assists with post-mortem care when needed.
  • Ensures residents have fresh ice water by refilling pitchers with ice and/or water at least once every shift.
  • Cleans water pitchers, cups, bedpans, and urinals per schedules.
  • Rinses personal clothing, sheets, etc. if soiled before placing in appropriate laundry barrel.
  • Assists residents with bedpan or urinal.
  • Empties catheter bags at end of shift and records output on appropriate record.
  • Assists with rehabilitation programs such as bowel and bladder retraining, grooming, etc., and charts on Rehabilitation sheet every shift.
  • Assists charge nurse with resident needs and treatments as directed.
  • Keeps a clean towel and a clean washcloth for each patient in bathrooms at all times, replacing immediately when used.
  • Empties incontinent pad barrel and linen containers every shift and replaces plastic bags in containers.
  • Reports at end of shift to oncoming CNAs.
  • Understands and agrees to follow policy of no transfers until certified.
  • Agrees to obtain assistance from a CNA assigned to assist with tasks they are not comfortable or trained to perform and with all transfers.

Requirements

  • Must be 18 years of age or older and have a high school diploma or equivalent.
  • Must be a graduate of a NA1 training program.
  • Must have passed the program and must pass state test within 4 months from completion.
  • Must be willing to care for geriatric patients and have a genuine concern for their welfare.
  • Must be willing to receive, understand, and follow orders.
  • Must be willing to work and cooperate well with other nursing staff, other departments, and families and visitors.
  • Must be kind and respectful to patients, staff, and visitors.
  • Must be able to use equipment without jeopardizing the safety of self, others, or equipment and use supplies safely and economically.
  • Must read, know, and follow personnel, department, and facility policies and procedures.
  • Must attend in-service training and other staff meetings as required.
