About the position

The Personal Care Assistant (PCA) role at Integrity Hearts Health Services LLC involves delivering patient-centered care to individuals with disabilities, the elderly, and those with chronic health conditions. The PCA is responsible for following care plans, assisting with activities of daily living, and ensuring a safe and therapeutic environment for clients. This position requires effective communication and documentation skills, as well as the ability to monitor and report changes in clients' conditions.

Responsibilities

  • Ensuring a safe and therapeutic environment for clients.
  • Monitoring clients' conditions and reporting changes to the Clinical Nursing Director.
  • Answering client questions about self-care techniques and providing instructions to assist clients and their families.
  • Performing light homemaking assistance, including personal laundry, meal preparation, and food shopping.
  • Making home visits to clients living in designated territories.
  • Accurately documenting observations, interventions, and evaluations related to client care management.
  • Taking and recording vital signs such as temperature, pulse, respiration, blood pressure, and oxygen levels.
  • Accompanying clients to recreational/social events and appointments.
  • Participating in the Care Team by providing input and suggestions.
  • Ensuring service delivery in accordance with company policies and industry standards.
  • Monitoring supplies and resources for client care.
  • Carrying out assigned tasks on clients and following Home Care Plans.
  • Using mechanical lifting devices for client transfers.
  • Assisting with activities of daily living including bathing, dressing, toileting, feeding, and ambulation.
  • Collecting and documenting patient data, including vital signs and changes in patient condition.
  • Providing accurate and timely documentation for patient care.
  • Reminding clients about medication schedules.
  • Providing respite care services.

Requirements

  • High School Diploma or GED equivalent.
  • Current personal care aide certification in good standing in the state of service.
  • Basic Life Support (BLS) certification from the American Heart Association.
  • A valid state driver's license or identification card and automobile liability insurance if applicable.
  • A minimum of one year of clinical experience is preferred.

Nice-to-haves

  • Experience in caregiving for at least one year is preferred.
