The Personal Care Assistant (Direct Support Professional) at Sevita is responsible for providing direct support to individuals with intellectual and developmental disabilities in residential settings. This part-time role involves assisting with daily living activities, health maintenance, and socialization, ensuring that individuals receive the care and support they need to live fulfilling lives. Responsibilities

  • Provide direct support or assistance in accordance with individual service or program plans.
  • Assist with socialization, health maintenance, medication administration, and skill or behavioral development.
  • Help individuals with personal care, housekeeping, recreational activities, transportation, and community integration.
  • Document progress and activities accurately and maintain confidentiality.
  • Assist with money management and ensure the safety of individuals' funds and property.
  • Maintain healthy and professional relationships with individuals, families, and case managers.
  • Report any instances of alleged abuse or neglect and document incidents as required.
  • Accompany individuals to medical appointments and relay information to medical providers.
  • Administer medications and treatments as assigned, ensuring proper documentation and security of medications.
  • Monitor individuals' health and communicate concerns to supervisors or medical personnel.
  • Transport individuals safely into the community and maintain a clean and safe environment.

Requirements

  • High school diploma or equivalent preferred.
  • Six months of experience in human services preferred.
  • Must be 18 years of age.
  • Valid driver's license in good standing, car registration, and vehicle insurance if providing transportation.

Nice-to-haves

  • Experience working with individuals with disabilities.
  • First aid and CPR certification.
