About the position

The Personal Care Assistant (PCA) at Shine Bright Care is responsible for providing ongoing support and services to clients with various needs in their homes or communities. The role focuses on ensuring client safety, working towards individualized goals, and maintaining effective communication with team members. The PCA will engage in professional relationships while adhering to service plans and documentation requirements.

Responsibilities

• Provide ongoing services to clients and individuals with needs in their community or home.
• Ensure the safety of individuals and work on individualized goals.
• Document daily activities and maintain comprehensive records.
• Implement the Service Plan and ensure compliance with productivity standards.
• Promote a strengths-based perspective to empower clients.
• Develop individualized Service Plans in collaboration with families and team members.
• Monitor the delivery of services to ensure they meet service plan goals.
• Coordinate transportation for service recipients.
• Engage with community resources to support client needs.
• Assist individuals in transitioning through various life domains and levels of care.
• Maintain regular and reliable attendance and punctuality for scheduled events.

Requirements

• Must be at least 18 years of age.
• Knowledge of HIPAA laws to safeguard confidential records.
• Strong interpersonal and customer service skills.
• Effective written and oral communication skills.
• Ability to maintain a confidential working relationship.
• Experience with documenting in the Therap database is preferred.
• Ability to follow oral and written instructions.

Nice-to-haves

• Confirmed service in the industry with advanced training in behavioral, medical, or caregiver services is preferred.
