About the position

The Personal Care Assistant (Direct Support Professional) at D&S Community Services is responsible for providing direct support to individuals with intellectual and developmental disabilities. This role involves assisting with daily living activities, health maintenance, and community integration, ensuring that individuals receive the care and support they need to live fulfilling lives. The position requires a commitment to the organization's mission of delivering high-quality services and fostering positive relationships with those served.

Responsibilities

• Provide direct support in accordance with individual service or program plans.
• Assist with personal care, housekeeping, recreational activities, and community integration.
• Document progress and activities accurately and maintain confidentiality.
• Assist with money management and ensure the safety of individuals' funds and property.
• Maintain healthy relationships with individuals, families, and case managers.
• Report any instances of alleged abuse or neglect according to policy.
• Accompany individuals to medical appointments and relay information to medical providers.
• Administer medications and treatments as assigned, ensuring proper documentation and security.
• Monitor individuals' health and communicate concerns to appropriate personnel.
• Transport individuals safely into the community and maintain a safe environment.

Requirements

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Valid driver's license in good standing and vehicle insurance if providing transportation.
• Completion of all state-required training and certification in mandated timeframes.

Nice-to-haves

• Experience working with individuals with disabilities.
• First aid and CPR certification.
