About the position

We are looking for compassionate and caring Personal Care Workers to join our LIFE Broad Street team. LIFE Broad Street Healthcare is a member of Trinity Health PACE, dedicated to providing comprehensive care to the elderly through a coordinated care model. This model encompasses preventive, primary, acute, and long-term care services, all designed to assist older individuals in living as independently as possible while remaining within their community. The Personal Care Assistant will primarily work at the LIFE Broad Street facility, with occasional home care responsibilities as needed. This position qualifies for a $2500 sign-on bonus, contingent upon a two-year commitment, with payments distributed in three installments over this period. The role requires providing personal care assistance to maintain the independence of participants in conducting activities of daily living, which includes toileting, showers, eating, and grooming. Additionally, the Personal Care Assistant will assist participants in maintaining physical activity, exercise, and walking. Behavioral supervision or support is also a key responsibility, which includes providing reality orientation, reminders for nursing and medication instructions, diet restrictions, and emotional reassurance. The position also involves engaging with participants through conversation, recreational activities, meal and snack serving, and housekeeping tasks. Responsibilities

  • Provide personal care assistance to maintain the independence of participants in daily living activities such as toileting, showers, eating, and grooming.
  • Assist participants in maintaining physical activity, exercise, and walking.
  • Provide behavioral supervision or support for participants, including reality orientation and reminders for nursing and medication instructions.
  • Engage in general activities with participants, such as conversing, recreation activities, meal and snack serving, and housekeeping.

Requirements

  • High school diploma or equivalent required.
  • Active Home Health Aide certification in the state required.
  • Certified Nurse Assistant Certification preferred.
  • Active CPR Certification required.
  • Minimum of one (1) year of documented experience working with a frail or elderly population, or appropriate training from the PACE organization if less than one (1) year of experience.

Nice-to-haves

  • Experience in senior care is preferred.
