As a member of our PACE Center team, you will play a crucial role in assisting participants with personal care, restorative rehabilitation, and engaging them in both group and individual activities. Your responsibilities will include greeting participants upon their arrival, helping them settle in for the day, and providing supervision and encouragement as needed. You will also assist with meal service and cleanup, ensuring a welcoming environment for all participants. In this position, you will actively contribute to the operations of the Day Center by sharing ideas for activities and suggesting improvements to daily operations. You will be involved in participant care planning and assessment, reporting observations to the Activities Coordinator, PACE Center Nurses, and Rehabilitation staff. This role requires flexibility, as you may need to float to other sites to support patient care needs across the program. Your daily tasks will include preparing participants for departure, accompanying them on transportation to ensure their safety, and assisting with activities of daily living (ADLs) as outlined in their care plans. You will also carry out simple treatments under the direction of nursing staff and assist with group and restorative activities, such as arts and crafts, exercise groups, and cognitive activities. Documentation of participant activities and communication of any changes in behavior or mood will be essential to ensure quality care. Maintaining cleanliness and order of supplies and equipment is a key aspect of this role, as is ensuring safety in food handling and serving. You will implement prescribed restorative therapy and exercise programs, demonstrate proper transfer techniques, and utilize fall prevention strategies. Your commitment to maintaining a clean and safe environment in the PACE Center and participant apartments will be vital to the well-being of our participants.
