As a Personal Care Assistant at NeighborHealth, you will assist PACE Center participants with personal care, restorative rehabilitation, meals, and both group and individual activities. Your role will include accompanying participants on transportation as needed to ensure their safety. You may also be assigned to assist supported housing participants with morning care. Your contributions will be vital in the operations of the Day Center, where you will be encouraged to share ideas for the activities program and suggest improvements for daily operations. You will play a key role in participant care planning and assessment by providing reports to the Activities Coordinator, PACE Center Nurses, and Rehabilitation staff. This position requires flexibility, as you may need to float to other sites to support patient care needs across the program. You will greet participants upon arrival, help them settle in for the day, and assist with serving coffee and meals. Providing supervision and encouragement will be essential, as will attending daily morning reports and reporting any participant concerns to the appropriate staff members. Your responsibilities will also include preparing participants for departure in the afternoon, assisting with clothing, and escorting them to the vans. You will accompany participants on the van to ensure their safety during transport when necessary. Assisting participants with Activities of Daily Living (ADLs) as defined in their care plans will be a significant part of your role, including toileting assistance when requested. You will also accompany participants to appointments as assigned and carry out simple treatments under the direction of nursing staff, such as catheter care and skin checks during personal care. In addition, you will assist participants with group and restorative activities under the direction of the Recreational Therapist, which may include arts and crafts, exercise groups, cognitive activities, and music groups. Documenting participant activities in accordance with procedures and communicating any observations regarding changes in behavior or mood will be crucial. You will also help maintain cleanliness and order of supplies and equipment, and contribute suggestions for beneficial supplies or equipment for the recreation program. Your role will involve ensuring safety in food handling and serving, following dietary restrictions, and implementing prescribed restorative therapy and exercise programs under the direction of licensed rehabilitation or nursing staff. You will demonstrate safe and proper transfer techniques and utilize fall prevention techniques as outlined in care plans. Overall, your role as a Personal Care Assistant will be integral to the well-being and care of our participants at NeighborHealth.
