About the position

The Personal Care Assistant at Woodholme Gardens is responsible for providing quality care to residents in a respectful and compassionate environment. This entry-level position involves following service plans, documenting changes in residents' conditions, and ensuring compliance with safety and quality standards. The role emphasizes teamwork and communication among staff to meet the needs of residents effectively.

Responsibilities

• Follow the services outlined in each resident's service plan in a friendly, respectful, and supportive manner.
• Document any observed change of condition in accordance with applicable laws and company policies.
• Comply with the Continuous Quality Improvement Program and ensure that services delivered are complete and tailored for each resident.
• Report identified issues not supporting quality services to the immediate supervisor for correction.
• Understand and follow all company policies as well as local, state, and federal rules and regulations as they apply to services delivered for all residents.
• Work in a safe manner in conjunction with other employees and report any unsafe actions immediately to the supervisor.
• Foster teamwork with co-workers and assist others beyond job responsibilities.
• Communicate between Personal Care Assistants on resident needs.
• Perform other duties as assigned or needed.

Requirements

• Entry-level position with no prior experience required.
• Ability to follow service plans and document changes in resident conditions.
• Understanding of HIPAA, OSHA, and Blood Borne Pathogen Guidelines.
