About the position

The Staff Pharmacist - Outpatient will fill orders with accuracy; check technician prepared products before release; maintain security of pharmacy and controlled substances; enter orders and patient information with accuracy; aid in the training of new staff; update the Manager about the daily activities of the pharmacy; aid the Manager in maintaining the pharmacy and all other duties as assigned.

Responsibilities

  • Utilizes the pharmacy computer system to fulfill job duties.
  • Enters new drug orders into computer profile.
  • Checks all ordered drug against reported patient allergies.
  • Reviews existing patient drug therapy regarding potential drug interactions with proposed new drug.
  • Enters all patient demographic information.
  • Enters correct charge descriptions.
  • Enters correct medication, including description, strength, route, expiration date, and start/stop dates.
  • Verifies all technician computer entries.
  • Corrects all orders which are entered in error or may be misunderstood.
  • Posts debits/credits accurately.
  • Enters metric dosage.
  • Maintains order entry error rate <2%.
  • Maintains a serious order entry error rate <0.5%.
  • Generates required reports.
  • Alerts Director of any problems with system software or hardware.
  • Prepares and dispenses prescriptions to patients or supervises qualified pharmacy technician in these functions.
  • Insures the appropriateness of drug dose and frequency for each medication ordered before dispensing.
  • Clarifies any ambiguities with prescriber.
  • Reviews all direct copies of physician orders for appropriateness.
  • Fills orders with correct medication.
  • Prioritizes work to prepare stat, now and waiting in order of importance.
  • Follows through on incomplete prescription orders to ensure medication has been ordered for the next day.
  • Fills discharge prescriptions and requisitions from nursing floors in a timely fashion.
  • Checks for computer entry errors and corrects if appropriate.
  • Checks cells/cassettes/canisters/robot for accuracy if filled by a technician as applicable.
  • Maintains records as required by law to document drug transactions within the Pharmacy and throughout the hospital if appropriate.
  • Initiates and terminates controlled drug sign-out sheets correctly.
  • Ensures investigational drug logs and perpetual inventories are accurate and current.
  • Completes and signs End-of-Day logs for all prescriptions processed on a daily and/or monthly basis as required by Pharmacy policy.
  • Completes notification reports regarding medication errors or other incidents.
  • Maintains Education Log.
  • Orders medications when stock is low.
  • Keeps Director informed of abnormal use of items.
  • Loans and borrows medications per policy.
  • Safely uses departmental equipment.
  • Disposes of waste in the proper containers.
  • Demonstrates fire safety verbally.
  • Performs unit inspections monthly if necessary.
  • Performs outdate checks on a monthly basis.
  • Assists in training of new staff.
  • Supervises all technician activity within the pharmacy.
  • Maintains cash register and money under observation and cashes out daily and completes reports or assigns and supervises technicians as needed.
  • Maintains security of pharmacy and controlled substances.
  • Supervises activities of support staff effectively.
  • Provides Director with feedback regarding activities of support staff.
  • Works with other pharmacists and technicians to settle work priority disputes.
  • Informs oncoming staff of pending problems, physical call backs, etc.
  • Reviews patient profile to ensure appropriate and safe drug therapy.
  • Prevents significant drug interactions and avoidable drug allergy interactions.
  • Reviews patient chart for requisite background information prior to contacting physician.
  • Keeps current on new drugs and answers questions timely and accurately.
  • Participates in drug inventory control through formulary management.
  • Responsible for maintaining the pharmaceutical quality assurance standards.
  • Provides assistance to the Director of Pharmacy in attending specified committees of the hospital when requested.
  • Completes special projects on time.
  • Willingly completes other duties as assigned or requested.

Requirements

  • Graduate of a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE).
  • Current license in state of employment.
  • One to two years' retail/outpatient pharmacy experience preferred.
  • Retail chain experience preferred.
  • Knowledge of patient counseling, drug therapy, and drug-drug interactions.
  • Certifications in Immunization and Medication Therapy Management a plus.
  • Working computer knowledge.
  • Effective verbal and written communication skills.
  • Must be able to perform the essential functions of the position with or without reasonable accommodation.
