About the position

The Pharmacist at OnePoint Patient Care plays a crucial role in delivering high-quality care and support to patients, caregivers, and families. This position involves dispensing medications, providing clinical advice, and managing delivery services, ensuring that patients receive the best possible pharmaceutical care.

Responsibilities

• Provides clinical consultations to patients as well as recommendations to nurses, physicians, and other hospice personnel
• Evaluates medication profiles, compliance, and modifications to achieve desired therapeutic outcomes
• Answers customer questions and resolves customer issues in a timely manner
• Retrieves data in the pharmacy system
• Manages medication orders
• Ensures proper storage and security conditions for medications
• Maintains a productive, orderly, and error-free work environment

Requirements

• Doctor of Pharmacy Degree or Bachelor of Science Pharmacy Degree
• Current and good standing Pharmacist License
• Strong communication skills, customer service, and problem-solving skills
