About the position

The Pharmacist position at OnePoint Patient Care is a PRN role focused on delivering high-quality pharmaceutical care to patients, caregivers, and families. The Pharmacist will be responsible for dispensing medications, providing clinical consultations, and ensuring the proper management of medication orders and storage.

Responsibilities

• Provides clinical consultations to patients and recommendations to nurses, physicians, and other hospice personnel.
• Evaluates medication profiles, compliance, and modifications to achieve desired therapeutic outcomes.
• Answers customer questions and resolves customer issues in a timely manner.
• Retrieves data in the pharmacy system.
• Manages medication orders.
• Ensures proper storage and security conditions for medications.
• Maintains a productive, orderly, and error-free work environment.

Requirements

• Doctor of Pharmacy Degree or Bachelor of Science Pharmacy Degree
• Current and good standing Pharmacist License
• Strong communication skills
• Customer service and problem-solving skills
