About the position

The Pharmacist at Heart Hospital of Austin is responsible for ensuring the safe and effective use of pharmaceuticals in patient care. This role involves monitoring medication therapy, providing drug information, and collaborating with healthcare providers to achieve optimal patient outcomes. The Pharmacist will also be involved in compounding and dispensing medications, as well as maintaining the hospital formulary.

Responsibilities

  • Monitor and evaluate the use of pharmaceuticals for patient care.
  • Interact with patients and healthcare providers to ensure compliance with standards of practice.
  • Interpret medication orders and transcribe them into the computer system.
  • Provide accurate medication therapy and compound pharmaceuticals.
  • Monitor drug therapy regimens for contraindications and interactions.
  • Suggest cost-effective therapeutic medication alternatives to medical staff.
  • Sustain the hospital formulary and promote rational drug therapy selection.
  • Assist with clinical services such as drug consults and anticoagulant monitoring.
  • Complete notification reports regarding medication errors and adverse drug reactions.

Requirements

  • Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD).
  • Current Pharmacist license with Texas State Board of Pharmacy (TSBP).
  • Previous hospital experience is preferred.
