About the position

The Pharmacy Clinical Specialist at Houston Methodist is responsible for advanced clinical pharmacy duties that ensure optimal pharmacotherapy for patients. This role involves conducting medication regimen reviews, supporting pharmacy protocols, and providing education to healthcare staff. The specialist also precepts pharmacy students and residents, contributing to the organization's mission of delivering innovative and cost-effective pharmaceutical care.

Responsibilities

  • Perform advanced clinical pharmacy duties to ensure optimal pharmacotherapy for patients.
  • Conduct daily medication regimen reviews for designated patient populations.
  • Support pharmacy managed protocols and medication reconciliation activities.
  • Educate fellow pharmacists, nurses, and medical staff on pharmaceutical care.
  • Precept pharmacy students and residents during clinical rotations.
  • Collaborate with healthcare professionals to manage drug therapy effectively.
  • Provide patient-specific and non-patient specific drug information support.
  • Participate in quality management initiatives and medication use evaluations.
  • Contribute to pharmacoeconomically sound medication therapy by adhering to formulary standards.
  • Develop and present educational materials and programs for pharmacy staff.

Requirements

  • Graduate of an education program approved by the credentialing body for required credentials.
  • Successful completion of an ASHP-accredited PGY1 residency or equivalent experience (3 years of pharmacy experience per residency year) required; PGY2 or fellowship preferred.
  • Three years of clinical practice experience may substitute for residency.
  • Must meet HM standards of an ASHP-Qualified Residency Preceptor within one year of employment.
