About The Position

The Pharmacy Liaison plays a crucial role in providing integrated support to patients and pharmacists, ensuring high-quality patient care through medication management services. This position requires strong interpersonal skills to establish relationships with patients and healthcare professionals, while also being detail-oriented and organized to manage various administrative tasks effectively. The role is centered around enhancing patient adherence to medication regimens and collaborating closely with clinic teams to deliver exceptional service.

Requirements

  • State registered as a pharmacy technician.
  • 2 years of experience as a Pharmacy Technician is required; 3-5 years preferred.
  • High School Diploma or GED required.
  • Strong interpersonal communication skills and ability to work independently.
  • Proficient in Microsoft Office, particularly Excel and Word.

Nice To Haves

  • Nationally certified as a CPhT or equivalent is preferred but not required.
  • Knowledge of transplant, hepatitis C, infectious disease (including HIV), and/or oncology.

Responsibilities

  • Provide 'legendary' care to hospital patients by delivering custom medication management services.
  • Work seamlessly alongside hospital clinicians to serve patients both face-to-face and over the phone.
  • Educate prospective pharmacy patients on how the hospital improves their overall care.
  • Support outpatient retail and specialty prescription medication needs.
  • Grow the specialty pharmacy's patient population.
  • Ensure patients remain on track with medication regimens through outbound therapy/medication adherence check-ups and securing refill prescriptions.
  • Communicate effectively with the centralized Patient Service Center regarding refill reports and appointment lists.
  • Help develop new processes to ensure smooth operations and patient care.
  • Identify medications requiring special storage and handling, such as hazardous or refrigerated medications.
  • Answer, resolve, and triage inbound inquiries.
  • Perform refill reminder calls under the supervision of a pharmacist.
  • Resolve insurance-related issues, including prior authorizations, and assist patients with financial assistance forms.
  • Ensure appropriate documentation in all required systems and tracking mechanisms.
  • Manage, organize, and update relevant data using database applications.
  • Identify trends, resolve problems, recommend improvements, and implement changes.
  • Actively participate in process improvement initiatives.
  • Act as a value-added business partner to stakeholders throughout the organization.
  • Protect the organization's value by keeping information confidential.
  • Update knowledge by participating in educational opportunities and maintaining professional networks.
