Pharmacy Medication Advisor (SCAN Temp)

About the position

The Pharmacy Medication Advisor at SCAN Health Plan is responsible for conducting telephonic outreach to members identified as high risk for medication adherence issues. This role involves assessing barriers to medication adherence, collaborating with various stakeholders to ensure members receive quality care, and documenting outreach efforts in the care management system. The position aims to support pharmacy quality measures and enhance the overall health outcomes of seniors.

Responsibilities

• Conduct telephonic outreach to SCAN members with identified medication adherence issues.
• Assess barriers to medication adherence and perform necessary follow-up with stakeholders.
• Document outreach information in the care management system during phone calls.
• Communicate with stakeholders via phone, email, and fax regarding health interventions and quality initiatives.
• Utilize department procedures, workflows, and training materials to support member adherence.
• Identify barriers to work processes and report them to the supervisor/manager.
• Maintain member privacy and confidentiality of information.
• Adhere to quality, compliance, and regulatory standards to achieve organizational outcomes.
• Support the achievement of SCAN's Vision and Goals.
• Contribute to team efforts by accomplishing related results as needed.

Requirements

• Pharmacy Technician license in good standing (strongly preferred).
• 2-3 years of medical assessment experience or related healthcare experience (preferred).
• Demonstrated knowledge of assessment, health, and functional problems of older adults and managed care.
• Strong interpersonal skills, including excellent written and verbal communication skills.
• Strong organizational skills and ability to multitask.
• Ability to maintain confidentiality appropriately.
• Strong analytical and critical thinking skills.
