About the position
The Pharmacy Medication Reconciliation Technician plays a crucial role in supporting pharmacy services by obtaining medication and allergy histories for patients admitted to the hospital. This position involves direct interaction with patients and their families, as well as communication with pharmacies and healthcare providers to ensure accurate medication management during hospitalization and post-discharge.
Responsibilities
- Obtain medication and allergy histories for patients being admitted to the hospital.
- Interview patients and their families to gather necessary information.
- Contact pharmacies, long-term care facilities, and other providers to verify medication histories.
- Communicate collected information to pharmacists and healthcare providers to assist in treatment plans and medication orders.
- Support pharmacy services in ensuring accurate medication reconciliation.
Requirements
- Pharmacy technician experience
- Pharmacy Technician Certification
- Registration with Indiana Board of Pharmacy within 30 days of hire date (for new registrants) or active registration upon hire (for previously registered)
- Nationally certified as a pharmacy technician
- Minimum 6 months of pharmacy experience
Nice-to-haves
- Medication history certificate preferred
