About the position

The Pharmacy Tech-Medication Access Specialist plays a crucial role in assisting healthcare providers by ensuring patients' compliance with their medications and maintaining accurate medication history information. This position involves direct communication with patients, families, and healthcare professionals to document and clarify medication details, ultimately contributing to improved patient care and safety.

Responsibilities

  • Obtain and clarify patients' home/chronic medication.
  • Use the EHR to input patients' home/chronic medication lists.
  • Communicate discrepancies identified via the medication history process with the pharmacist and make clarifications with the patient and prescribers as needed.
  • Direct all questions or inquiries from patients or families that are outside the scope of taking the medication history to the appropriate caregivers or staff.
  • Provide high quality customer service to patients, family members, and care team members.
  • Adapt behavior to the specific patient population, including respect for privacy and communication style.
  • Perform other related duties as required.

Requirements

  • High School Diploma or equivalent.
  • 2 years of experience as a pharmacy technician.
  • Medication History Certificate from the American Society of Health-System Pharmacists (ASHP) must be obtained within 6 months.
  • Proficiency in using computers, software, and web-based applications.
  • Knowledge of pharmaceutical mathematics.
  • Effective verbal and written communication skills.

Nice-to-haves

  • Experience with taking patient medication history.
