About the position

ProHealth Care is seeking a dedicated Pharmacy Technician for Medication Reconciliation to join our team. This position is part-time at 0.75 FTE and involves assisting pharmacists, physicians, and nurses in the medication reconciliation process. The primary goal is to ensure that any discrepancies between medications taken prior to a patient's admission and those ordered upon admission are intentional and do not lead to negative patient incidents. The role also includes assisting with the reconciliation process at discharge in coordination with pharmacists and case managers, as well as offering discharge prescription services through ProHealth Retail Pharmacy. The Pharmacy Technician will be responsible for collecting appropriate insurance and demographic data for prescription processing, ensuring a smooth transition for patients as they leave the hospital. The Pharmacy Technician will work 10-hour shifts, every third weekend, with varying start times between 6 AM and 10:30 AM. This role is essential in promoting the hospital's retail pharmacy program and ensuring that our patients receive the best possible care during their hospital stay and upon discharge.

Responsibilities

  • Assists pharmacists, physicians, and nurses with medication reconciliation processes.
  • Completes medication histories for new admissions.
  • Offers discharge prescription service through ProHealth Retail Pharmacy.
  • Collects appropriate insurance and demographic data for prescription processing.
  • Promotes the hospital's retail pharmacy program through offering discharge prescription services.
  • Provides service to customers to meet or exceed their needs and expectations.
  • Collects and compiles data, statistics, and other information needed for quality assurance programs or reports.
  • Participates in the team approach to quality improvement to support the Pharmacy Department's commitment to Total Quality Improvement.
  • Maintains updated knowledge and skills and contributes to the education of others.
  • Participates in planning processes by establishing personal goals to support quality improvement efforts.

Requirements

  • High School Diploma or equivalent
  • Pharmacy Technician (CPhT) Registered with the State of Wisconsin
  • Valid WI driver's license and in good standing
  • Basic Life Support Certification (BLS)
