The Pharmacy Technician for Medication Reconciliation at ProHealth Care plays a crucial role in assisting healthcare professionals with the medication reconciliation process. This position ensures that discrepancies between pre-admission and admission medications are intentional and safe for patients. The role involves collaboration with pharmacists, physicians, and nurses, as well as providing discharge prescription services and collecting necessary data for prescription processing.

Responsibilities

  • Assists pharmacists, physicians, and nurses with medication reconciliation processes.
  • Completes medication histories for new admissions.
  • Promotes the hospital's retail pharmacy program through discharge prescription services.
  • Provides service to customers to meet or exceed their needs and expectations.
  • Collects and compiles data, statistics, and other information for quality assurance programs.
  • Participates in the team approach to quality improvement in the Pharmacy Department.
  • Maintains updated knowledge and skills and contributes to the education of others.
  • Participates in planning processes by establishing personal goals to support quality improvement efforts.
  • Provides age/developmentally appropriate care in accordance with Age/Developmental Specific Care Guidelines.

Requirements

  • High School Diploma or equivalent
  • Pharmacy Technician (CPhT) certification
  • Registered with the State of Wisconsin
  • Valid WI driver's license in good standing
  • Basic Life Support Certification (BLS)
