About the position

The Physical Therapist position at San Gorgonio Memorial Hospital Outpatient Physical Therapy in partnership with Loma Linda Health system involves providing comprehensive care in an outpatient setting. The role focuses on establishing individualized treatment plans under physicians' orders, conducting evaluations, and coordinating care with other medical staff to enhance patient recovery and education.

Responsibilities

• Establish and maintain a comprehensive, individualized interdisciplinary plan of care under Physicians' orders.
• Perform evaluations and integrative treatment planning.
• Set patient-specific functional goals and conduct follow-up assessments.
• Provide ongoing patient and family education.
• Coordinate the patient's Plan of Care with other Medical and Rehabilitation Staff.
• Maintain EMR in accordance with departmental policies and procedures, JCAHO standards, and regulations.
• Foster a constructive and supportive approach to department operations.
• Participate in process improvement and quality improvement measures.
• Provide appropriate supervision and support to Physical Therapy Assistants and Rehabilitation Technicians.
• Act as a clinical educator for students and other clinical staff.

Requirements

• Graduation from an accredited program of physical therapy or foreign-trained physical therapy with State Board Licensure.
• Current California Board of Physical Therapy licensure.
• BLS certification.
