About the position

UPMC Home Healthcare is seeking a Physical Therapist to provide in-home care for patients in Beaver County. This full-time position involves establishing treatment plans, collaborating with healthcare teams, and ensuring optimal patient care while adhering to regulatory standards. The role includes responsibilities such as supervising assistants, maintaining documentation, and participating in on-call coverage, all within a supportive environment that prioritizes employee well-being.

Responsibilities

• Perform duties in alignment with UPMC Home Healthcare's service management philosophy.
• Participate in on-call coverage rotation for the agency.
• Assign and supervise the Physical Therapy Assistant according to state guidelines.
• Advise and consult with families, interdisciplinary team members, and physicians to ensure optimal patient care.
• Maintain and complete all documentation promptly and accurately, including visit assessments and regulatory standards.
• Establish the plan of care in collaboration with the physician and patient based on physician's orders and patient needs.
• Evaluate, plan, and treat patients using effective physical therapy programs and diagnostic testing.
• Make recommendations and order necessary durable medical equipment (DME) when indicated.
• Provide age-sensitive professional physical therapy care in the home setting.

Requirements

• Graduate from a physical therapy curriculum approved by relevant accrediting bodies.
• Licensure as a Physical Therapist (PT).
• Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification.
• Valid Driver's License.
• Automotive Insurance.
