About the position

The Physical Therapist at JAG Physical Therapy is responsible for assessing patients' strengths and weaknesses to create effective treatment plans aimed at healing and maximizing functional independence. This role requires a compassionate and motivated individual who can maintain a positive environment and encourage patients in their recovery journey. The therapist will work closely with patients, utilizing their clinical knowledge to implement and execute treatment plans on a daily basis.

Responsibilities

  • Collect and record patient care data following guidelines and regulations.
  • Prepare for evaluations by obtaining patient history and understanding diagnoses.
  • Select and administer tests and measures to formulate diagnosis and treatment plans.
  • Establish treatment plans based on evaluation results and best practices.
  • Collaborate with interdisciplinary teams and patients' support systems.
  • Assess and modify treatment plans and goals as necessary.
  • Provide education and training to patients and their support systems.
  • Oversee support staff including PTA, ATC, and PT aides.
  • Promote JAG in the community through lectures and presentations.

Requirements

  • Bachelor's or Master's Degree required; Doctorate degree preferred.
  • Licensed Physical Therapist in the state of practice.
  • Completed state regulated CEU requirement in appropriate state (NY, NJ).
  • Valid CPR License.
  • Experience/knowledge with EMR software is preferred.
  • Excellent oral and written communication skills.
  • Strong clinical decision-making skills.
  • Excellent work ethic and dedication to patient success.
  • Basic computer skills.

  • Experience with EMR software.
