About the position
The Physical Therapist for Home Care at Mercy Home Health is responsible for providing skilled physical therapy services to homebound patients in the greater NE Philadelphia region. This role involves consulting, evaluating, planning, and administering therapy as prescribed by a physician, with the goal of restoring function, relieving pain, and preventing disability. The therapist collaborates with an interdisciplinary team to ensure quality care and may act as the patient care manager for a select group of patients, maintaining open communication and supporting a patient-centered care model.
Responsibilities
- Consult, evaluate, plan, and administer skilled Physical Therapy services to homebound patients.
- Collaborate with interdisciplinary team members to meet patient needs and achieve quality care.
- Manage and oversee primary patient care delivery when acting as the patient care manager.
- Maintain open communication with all members of the care group, both internal and external to the organization.
Requirements
- Graduate of an approved PT educational program (Bachelor's, Master's, or Doctorate preferred).
- Licensure as a Physical Therapist in the state of Pennsylvania.
- One year of experience as a professional therapist; home care experience is a plus.
- Must have a current driver's license and reliable transportation.
