About the position

At Mercy, we are dedicated to providing compassionate care and exceptional service through our healing ministry. We are currently seeking a Physical Therapist to deliver in-home physical therapy services to patients suffering from neuro and/or musculoskeletal disorders, under the guidance of a physician. This role is crucial in ensuring that our patients receive the highest quality of care in the comfort of their own homes. As a Physical Therapist, you will be responsible for assessing patients' needs, developing treatment plans, and implementing therapeutic interventions to promote recovery and improve quality of life. You will work independently, often in challenging situations, and must be able to adapt to changes in patient conditions and treatment protocols. Your ability to communicate effectively with patients, their families, and other healthcare professionals will be essential in fostering a collaborative care environment. In this position, you will be expected to manage your schedule efficiently, ensuring timely visits to patients while maintaining regular communication with the office to update your schedule. You will also participate in weekend and on-call rotations, which may require you to handle emergencies independently. The role demands a strong commitment to patient confidentiality and the ability to work well under pressure. Responsibilities

  • Provide in-home physical therapy services to patients with neuro and/or musculoskeletal disorders under the direction of a physician.
  • Assess patients' needs and develop individualized treatment plans.
  • Implement therapeutic interventions to promote recovery and improve quality of life.
  • Manage and schedule patient visits efficiently, ensuring timely care delivery.
  • Communicate effectively with patients, their families, and other healthcare professionals.
  • Participate in weekend and on-call rotations as required.
  • Handle emergencies independently and adapt to changes in patient conditions.
  • Maintain patient confidentiality and adhere to ethical standards.

Requirements

  • Graduate of an accredited physical therapy program.
  • Current state license to practice physical therapy.
  • Valid driver's license.
  • CPR certification.
  • Strong assessment and treatment planning skills.
  • Good time management and problem-solving skills.
  • Effective communication and documentation skills.
  • Ability to lift up to 50 pounds frequently and occasionally over 50 pounds.
  • Good physical health and ability to work independently.

Nice-to-haves

  • Experience in home health care settings.
  • Familiarity with neuro and musculoskeletal disorders.
  • Ability to work in hazardous conditions and follow written or verbal instructions.
